If you have requested an absentee ballot in prior elections and included a copy of your ID with the ballot request:
Fill in the form at MyVote.Wi.Gov or send a request on paper or by email or phone call to your municipal clerk’s office. You need not provide another copy of your ID.
If this is your first time requesting an absentee ballot or if you have moved or changed your name:
Fill in the form at MyVote.Wi.Gov or send a request on paper or by email to your municipal clerk’s office. You must also provide a photocopy of your acceptable photo ID*
If you are an “indefinitely confined”** voter:
Fill in the form at MyVote.Wi.Gov or send a request on paper or by email or phone call to your municipal clerk’s office. Select “indefinitely confined voter” when prompted. You are not required to provide a copy of your photo ID.
**Indefinitely confined means you are elderly or have a disability or medical condition that might make it hard for you to vote in-person at a polling place.
*Providing your ID: On a computer or tablet, take a photo of your photo ID and save it to your computer. If applying online at MyVote.Wi.Gov, upload the photo file when prompted. If using a smart phone with MyVote.Wi.Gov, at the prompt “add file,” select “take a photo,” Use your phone’s camera to photograph your ID; when the photo appears on screen, select “use photo” and “continue.” If using a paper application, print a copy of the ID and enclose it with your application. If applying by email, attach a photo of the ID to your email.
